🧪 Not sure which you were given? Look at what was in the box you lost — a plastic cup means urine, and a small lancet with a card to put drops on means blood. Tell us and we send the right one; guessing means a second delay.

Which kit are you replacing?

🔎 Can’t find it? Leave it blank and carry on. It only helps our laboratory tie your replacement to the kit that went missing — it is never a reason we cannot send you one. A kit you have lost is a kit you cannot read a number off.
